This website provides lab information and accepts replacement requests for review. A request is not a confirmed production order. Online payments are not yet active.
Information you submit
The replacement form collects patient name, date of birth, contact email, optional phone number, prescribing-office details, requested arches and quantities, screening answers, delivery address and optional notes. Please include only information needed for the request. Do not enter card details or upload clinical scans through email.
How requests are used
Request information is saved so authorized lab staff and the assigned prescribing clinician can review identity, suitable source records, work authorization, the individual quote, production and delivery. A request received email follows successful saving. No email-verification step or patient account is required to submit. Production still requires appropriate authorization, accepted pricing and verified payment.
Private status and necessary records
The browser where you submit receives a secure session cookie for private request status. The request reference or public case QR alone does not grant access. If your session expires or you use another browser, you can request a one-time secure access link at the email used for the request. That is an access-recovery option, not a prerequisite to submission.
Necessary request, authorization, quote, payment-reference and shipping records are retained for operating the service. Guest ordering does not mean no records are stored. A random retry reference may be kept in browser session storage to prevent duplicate submissions; patient form values are not stored there. Contact the lab about access, corrections or record-retention questions; specific retention periods and applicable obligations need confirmation by the business.
Office portal and source records
The partner portal has separate sign-in and office access controls. Clinical files and original cases are matched by authorized staff. New requests do not replace or erase those source records.
Contact and technical services
Contact buttons open drafts or copy the email address; they do not send messages automatically. Use general email for service questions, and signed-in case conversations for patient-specific clinical files. The hosting and connected storage/email services process the information necessary to operate these functions. Hosting may also process technical request information such as IP addresses and logs. No advertising trackers or external analytics have been added to this website.
Payment and service setup
Patient prices are individually quoted and separate from office wholesale fees. Payment and shipping-provider connections still require setup and verification. This description explains current software behavior; it is not a claim of legal, clinical or privacy compliance. The business must confirm appropriate notices, agreements and retention arrangements before expanding live services.
